Consequence-based risk thinking
Not all AI use cases carry the same risk. Drafting internal meeting notes is low-consequence — if the AI makes an error, someone corrects it before anyone is affected. Generating a legal contract clause is high-consequence — an error could create financial liability. Classifying patient symptoms is critical-consequence — an error could affect health outcomes.
Consequence-based thinking means matching the level of oversight to the potential impact of failure. Low-consequence tasks can be more automated with lighter review. High-consequence tasks require mandatory human review, verification processes, and clear accountability.
The mistake many organizations make is applying the same oversight level to everything. Either they treat all AI use as high-risk (which prevents adoption) or they treat all AI use as low-risk (which creates liability). The right approach is to classify each use case and match oversight accordingly.
Risk classification guide
Low: internal drafts, brainstorming, research synthesis. Medium: customer-facing communications, reports with business impact. High: legal documents, financial decisions, healthcare, hiring, compliance submissions. Classify before deploying.
Common AI risk categories
Accuracy risk: AI generates plausible but incorrect information (hallucinations). This is the most common AI risk and affects every generative AI application. The risk increases with the stakes of the output.
Privacy risk: sensitive data included in prompts may be stored, used for training, or accessible to the AI provider. Customer PII, financial data, proprietary information, and confidential communications should not be sent to external AI services without understanding data handling policies.
Bias risk: AI trained on historical data can reproduce and amplify human biases in hiring, lending, healthcare, insurance, and other areas where decisions affect people materially.
Dependency risk: over-reliance on AI can atrophy human skills, reduce critical thinking, and create operational fragility if AI services become unavailable or change terms.
Building a risk assessment habit
Before deploying AI for any business task, answer these questions: What is the worst outcome if the AI output is wrong? Who is affected? How would we know if it was wrong? What verification process exists? Who is accountable?
For low-consequence tasks, this assessment is a quick mental check. For medium and high-consequence tasks, document the assessment and review it with stakeholders. The goal is not to prevent AI use but to ensure the right safeguards match the right risks.
Review risk assessments periodically. As AI capabilities improve, some risks decrease. As AI use expands into new areas, new risks emerge. The assessment should evolve with your AI usage.

The press release that cited nothing
Situation
A startup used AI to generate a press release that included statistics about market size, growth rates, and competitor performance. None of the statistics were verified because the marketing team assumed the AI was pulling from reliable sources. A journalist fact-checked the release and found that several statistics were completely fabricated by the AI.
Analysis
The AI generated plausible-sounding statistics because that is what press releases typically contain. It did not research or verify anything — it predicted what statistics would fit the context. The marketing team treated a medium-consequence output (public-facing claims) with low-consequence oversight (no review).
